Patient Forms

We require that you fill out our patient forms located on the New Patient page of this website, and email, fax or mail the completed forms prior to scheduling an initial appointment. The paperwork provides us with detailed information about you and your health condition. Also, it is advised that you send your previous lab work and/or medical tests with your intake forms.

Prior to your first appointment, Dr. Mark will review your intake forms and lab work to become familiar with you and your health status. On the day of your appointment, he will ask questions and listen to you. Your first visit is designed for him to learn more about you, your condition, and your expectations to determine whether you are a candidate for this type of nutritional care. At the end of this appointment, he will determine what additional testing is required to further evaluate your health.

Health Analysis

You are required to have initial laboratory blood work as part of your evaluation. This blood work is analyzed on a functional level to help Dr. Mark rule out more serious conditions and to assist him in developing the most effective plan for you.  Instructions will be given to you on your first visit as to how to obtain proper blood work. Any additional testing, such as stool, saliva or urine, will be recommended based on your condition.

Report of Findings

Once we have received the recommended test results, Dr. Mark will review them along with your health history and previous labs. Next, he will give you a report of findings to answer the three most popular questions:

  • Can you help me?
  • What do you recommend that I do to get better?
  • How much is it going to cost?

Care Plan

It is necessary for us to have all the pieces of your puzzle before we can “put you back together.” Your care plan is based on your medical history, your diagnosis, the duration of your condition, and your present state of your health. Each care plan is designed on a case-by-case basis since everybody is created differently and responds differently. The outcome of your care depends on how well you follow the recommendations.
